O κερατόκωνος αποτελεί τη βασική αιτία µεταµοσχεύσεων κερατοειδούς παγκοσµίως. Στην Ελλάδα δεν έχει µελετηθεί στατιστικά η ενδηµικότητα του κερατόκωνου – οι µελέτες που έχει πραγµατοποιήσει η δική µας επιστηµονική οµάδα δείχνουν 1 στα 20 άτοµα να έχει κάποια «γεύση» κερατόκωνου, µια πραγµατικά πολύ εντυπωσιακή στατιστική αν αναλογιστεί κανείς ότι στην αγγλοσαξονική βιβλιογραφία αναφέρεται µόλις 1 στους 2.000 Αγγλοσάξονες να πάσχει από κερατόκωνο.
Ο κερατόκωνος, κατά την τελευταία εικοσαετία, αντιµετωπιζόταν ως µια πάθηση που δεν είχε θεραπεία, απλώς τον παρακολουθούσαµε.
Η εξέλιξή του µπορούσε να σταµατήσει σε ελαφρύ ή µέτριο βαθµό και το άτοµο µπορούσε να λειτουργήσει στην καθηµερινή του ζωή µε γυαλιά ή µε ειδικούς φακούς επαφής.
Αν η πάθηση συνέχιζε να εξελίσσεται ή αν υπήρχε δυσανεξία στους φακούς επαφής, χρειαζόταν να γίνει µεταµόσχευση κερατοειδούς. Η τεράστια αλλαγή – κλινικά – στην αντιµετώπιση του κερατόκωνου είναι η εισαγωγή της διασύνδεσης κερατοειδούς. Πρόκειται για µια επέµβαση που συνδυάζει σταγόνες ριβοφλαβίνης (βιταµίνη Β2) και υπεριώδες φως και έχει ως αποτέλεσµα να αυξάνει την τεκτονική σταθερότητα του κερατοειδούς σταµατώντας την εξέλιξη της νόσου και στις περισσότερες περιπτώσεις να κάνει και µια µικρή αντιστροφή της, δηλαδή να επιπεδώνει λίγο τον κώνο. Η διασύνδεση κερατοειδούς ήρθε αρχικά ως µια περιγραφή από τον γνωστό Γερµανό γιατρό Τheo Seiler. Ήµασταν η δεύτερη οµάδα στην Ευρώπη που ασχολήθηκε µε τη διασύνδεση κερατοειδούς, στην Αθήνα, το 2003. Από τα πρώιµα στάδια της έρευνας, κυρίως σε νεαρά αγόρια που επρόκειτο να υποβληθούν σε µεταµόσχευση κερατοειδούς, παρατηρήθηκε ότι ένα τεράστιο ποσοστό τελικά την απέφυγε.
Κατά τη διετία 2004-2006, εξελίξαµε και µοιραστήκαµε τα αποτελέσµατά µας µε την ελληνική και την παγκόσµια οφθαλµολογική κοινότητα µέσα από µια πληθώρα ανακοινώσεων και συνεδρίων. Η εντατική δουλειά που
πραγµατοποίησε η οµάδα µας, εξετάζοντας εκατοντάδες ασθενείς µε κερατόκωνο ετησίως, συνεισέφερε στις εξελίξεις της διασύνδεσης του κερατοειδούς. Αναγνωρίσαµε και εφαρµόσαµε τη διασύνδεση µεγαλύτερης ισχύος, άρα και µικρότερης χρονικής διάρκειας, αρχικά στα 6 mW, µετά στα 10mW, στα 15 mW και καταλήγοντας, τελικά, στα 30 mW, κάτι που εφαρµόστηκε παγκοσµίως. Συνδυάσαµε τη διασύνδεση κερατοειδούς µε µια τοπογραφικά κατευθυνόµενη σµίλευση, έτσι ώστε όχι µόνο να σταθεροποιηθεί ο κερατόκωνος, αλλά και να οµαλοποιηθεί ο κερατοειδής, µε την προοπτική να γίνει οπτικά λειτουργικός. Η τεχνική αυτή έγινε παγκοσµίως γνωστή ως το «Πρωτόκολλο της Αθήνας» και είναι αυτή τη στιγµή η πιο συνηθισµένη τεχνική που εφαρµόζεται για την αντιµετώπιση του κερατόκωνου. Αξίζει να σηµειωθεί ότι η οµάδα µας συµµετείχε στην εκπαίδευσητων πρώτων χειρουργών-οφθαλµιάτρων στις ΗΠΑ που ασχολήθηκαν µε αυτήν την τεχνική.
∆είξαµε in vitro, αλλά και κλινικά, ότι ο συνδυασµός της διασύνδεσης κερατοειδούς µε επεµβάσεις ρουτίνας, όπως η επέµβαση διόρθωσης µυωπίας καιυπερµετρωπίας LASIK, µπορεί να δώσει πολύ µακροχρόνια σταθερότητα στα διαθλαστικά αποτελέσµατα. Αυτό αποτέλεσε τη γέννηση της τεχνικής που είναι ευρέως γνωστή ως LASIK XTRA και, επίσης, ξεκίνησε από την Αθήνα.
Συµπεράναµε, επίσης, ότι σε περιπτώσεις τήξης κερατοειδούς, η φύση της διασύνδεσης συµβάλλει στην καθυστέρησή της και στην αποφυγή διάτρησης του κερατοειδούς από σοβαρές παθήσεις του εξωτερικού µέρους του οφθαλµού.
Το 2013 δε καταλήξαµε ότι από µόνη της η διασύνδεση του κερατοειδούς µπορεί να επιφέρει διαθλαστικές βελτιώσεις ακόµη και σε φυσιολογικούς οφθαλµούς, αν αυτή χρησιµοποιηθεί µε ειδική τεχνολογία. Σε αυτήν την περίπτωση, η δέσµη του υπεριώδους φωτός προβάλλεται στον οφθαλµό σύµφωνα µε συγκεκριµένα σχήµατα και µε διαφορετικές εντάσεις σε κάθε σηµείο στον κερατοειδή.
Σχεδόν δύο δεκαετίες µετά την πρώτη διασύνδεση του κερατοειδούς, η πρόωρη διάγνωση εξακολουθεί να είναι σηµαντική. Πιστεύουµε ότι η διασύνδεση πρέπει να θεωρηθεί το «εµβόλιο» για τον κερατόκωνο. Αν καταφέρουµε να διαγνώσουµε τον κερατόκωνο στο ξεκίνηµά του (στην εφηβεία), όπου κλινικά µπορεί να είναι εντελώς ασυµπτωµατικός, µπορούµε µε µια πολύ εύκολη παρέµβαση διασύνδεσης κερατοειδούς να σταµατήσουµε την εξέλιξή του σε αρχικό στάδιο. Η συγκεκριµένη παρέµβαση δεν απαιτεί αφαίρεση του επιθηλίου του κερατοειδούς (transepithelial high-fluence photorefractive cornea cross-linking).
Η οµάδα µας έχει αφιερώσει πολλά χρόνια εντατικής µελέτης σε χιλιάδες περιστατικά όπου διαφαίνεται ο τρόπος µε τον οποίο οι σύγχρονες µέθοδοι απεικόνισης του κερατοειδούς µπορούν να συµβάλουν στην πρόωρη διάγνωση του κερατόκωνου. Με κύριο κριτήριο την κερατοειδική συµµετρία αλλά και τις πολύ συναρπαστικές απεικονίσεις που κάνει πλέον η τεχνολογία OCT επιθηλίου, µπορούµε να αναγνωρίσουµε αλλαγές στην τεκτονική επίδοση του κερατοειδούς παρατηρώντας τις αλλαγές του επιθηλίου.
Θεωρούµε ότι ο έλεγχος για κερατόκωνο πρέπει να πραγµατοποιείται σε όλες τις ηλικίες. Για το λόγο αυτό, η επιστηµονική µας οµάδα εδώ και 15 έτη πραγµατοποιεί έλεγχο για κερατόκωνο σε κάθε ασθενή που εξετάζουµε και είναι πραγµατικά συγκλονιστικό το ποσοστό των ασθενών στην Ελλάδα που ανακαλύπτει τον κερατόκωνο µέσα από αυτή τη διαδικασία.
Μπορεί όσοι έχουν περάσει το 40ό έτος της ηλικίας τους να µην επηρεάζονται κλινικά από τον κερατόκωνο, καθώς στις περισσότερες περιπτώσεις δεν εξελίσσεται περαιτέρω, αλλά µέσω αυτού µπορούµε να κάνουµε διάγνωση του κερατόκωνου στα νεαρότερα µέλη της οικογένειας και έτσι να εξαλείψουµε τις σοβαρές µορφές προχωρηµένου κερατόκωνου και, κατ’ επέκταση, την ανάγκη να πραγµατοποιηθεί µεταµόσχευση κερατοειδούς στο µέλλον.
Η οµάδα µας είναι πεπεισµένη ότι, ως µέσο επιδηµιολογικής πρόληψης, θα έπρεπε να φωτογραφίζονται οι κερατοειδείς Ο έλεγχος γιακερατόκωνο πρέπει να πραγµατοποιείται σε όλες τις ηλικίες
όλων των µαθητών και µαθητριών της Α’ και της Β’ Λυκείου. Η διάγνωση θα µπορούσε να τους οδηγήσει στον χειρουργό-οφθαλµίατρό τους, όπου, συνδυάζοντας τον ευαίσθητο απεικονιστικό διαγνωστικό ελέγχο µε την εφαρµογή των νέων τεχνικών διασύνδεσης κερατοειδούς, µηδενίζεται ουσιαστικά η πιθανότητα της εξέλιξης της πάθησης και, κατά συνέπεια, η ανάγκη για µεταµόσχευση.
